Teen Volunter Program Application

Thank you for your interest in the Teen Volunteer Program. Please print all pages and read the application carefully before completing it.

This application has two parts:

  1. Application Form
    Complete the form, and on a separate sheet of paper, answer the Statement of Interest questions.
  2. Signature Form
    This form documents your interest in becoming a teen volunteer and should be signed by you, your parent/guardian, and your school guidance counselor or community service coordinator.

Put both parts of your application together in one envelope and mail it to:

Applicants will be selected for an interview based, in part, on the timely submission of application forms and thorough responses to the Statement of Interest questions.
