- High school attended: name, city, and state, zip code (if known), date of diploma or GED
- Colleges or universities attended: name, city, and state, zip code (if known)
- Major field(s) of study, type/year of degree(s) received (if no degree, show total credits earned and indicate whether these credits represent semester or quarter hours)
- College transcripts: submit a copy of your college transcript(s) only if requested in the vacancy announcement
