The print is of a left hand, and areas of all four fingers, the fingerprints that would have been added above each finger, the ball and inner edge of the palm, and part of the thumb have been blacked out. A black line crosses the center of what appears to be the page. Three sets of numbers and digits are in the lower right corner. “022676” and “000205” appear to have been stamped, and “DOD-044401” appears to be printed. The field with the handprint is titled “Palm Prints (Left Hand).” Above are fields for Name (Last, First, Middle), SSN, Case#, Signature, DOB, Date, and Signature of Official. All fields have been blacked out except the case number, “0114-04-CID083-66189,” the date of birth, “15 Jun 77,” and date, “22 Jun 04.” A note appears to be hand-written under the date, “(b)(6)2, (7)(c)-2.” The outlines of two circles at the top center resemble the distinct hole punch of official documents.
